If you are a new Advertiser on Affinity and wish to make payments through your PayPal Account, we request you to kindly fill out a Paypal Verification Form and send it across to us. PayPal Verification is a one-time security measure to preempt any un-authorized transactions through your PayPal Account. We value your business and request your cooperation with the following:
1. Kindly fill out this PayPal Verification Form - Click here to download (Hyperlink the click here link to the attached document)
2. Fax the executed form to +1-312-277-1021 or email it as an image attachment to billing@affinity.com
3. Provide a scanned copy of a valid government issued Photo ID.
NOTE: Filling out the PayPal Verification Form is mandatory for ALL Advertisers. Failing to do so will restrict us from processing your payments.
